Please follow these
instructions in order to apply for positions. Step 1. Fax copies of the following items to (586) 771-4205. 1. Current Resume and or 2 letters of recommendation from previous clients or families 2. Copy of Current BLS CPR Card-Healthcare Provider 3. Copy of Current Michigan CNA or HHA certificate 4. Copy of Current TB Test or CXR results with signs and symptoms sheet 5. Picture ID Drivers License and Current Insurance certificate if transporting clients 6. Print and complete the attached Skills Checklist and Home Care Test and employment documents. Step 2. Download, print, sign and fax back completed employment forms as listed below.
